WAIVER SERVICES

Hope Springs Farm participants are funded by their Medicaid Waiver funded benefits.  If your family member does not have a Medicaid Waiver benefit you should contact your local County MH/ID Office. We can also offer a private pay/tuition option for families who don't have a waiver.

ADMISSION/ REFERRAL PROCESS 

Our admission process starts with the individual, family, or the support Team considering a day services program for the individual. We offer facility-based and community-based programs.
Our facility-based program located at Hope Springs Farm in Hershey PA includes community outings to do volunteer, recreation, and life-skills activities.
Our community-based program is 100% community activities - Growers don't come to the Farm. They are picked up at home or they meet at community locations and spend their whole day in their community doing Volunteer, recreation, and life-skills activities.


STEP 1.  The family or supports coordinator should contact us to set up a tour for the individual their family and any Team members who would like to accompany them to see if Hope Springs Farm would be an appropriate option. You can complete and submit the Consent and Contact Forms prior to your visit or we can do them when we meet.

STEP 2.  When the Team agrees, the supports coordinator will make a referral and submit the necessary paperwork to the Admission Panel. Typically they are the ISP/IEP, any current behavior support plans, and anything the Team considers relevant for the Admission Panel to consider.

STEP 3.  The Admission Panel will prioritize all interested candidates creating a waiting list based on need and appropriateness of the proposed objectives and goals. The support Team will be notified when a vacancy opens for their individual.

We try to schedule Admission Panel Reviews 4 to 6 times per year, or more when we receive more referrals.

We have a variety of yarn for sale on Etsy. Our five alpacas- Nina, Mimi, Maddie, Chalupa, and Hope, and four sheep- Phoebe, Sarah, Mary, and Elvis provide the fiber for our yarn. Once a year, we shear our alpacas and sheep and send the fiber to a processor. It is processed at Gurdy Run Woolen Mill in Halifax, PA.
